There are three HEALTHMILES PLUS health insurance packages offering a range of coverage, deductible and savings options (or you can customize your own package). All three packages are PPOs (Preferred Provider Organization). Essentially, PPOs provide members with a network of doctors, hospitals and pharmacies that have agreed to offer their services at a discounted rate. Who you see is up to you. However, you get the most from your plan when you visit a network doctor.

So, whether you’re on vacation or at home, you and your family can see a doctor or specialist and get the prescription drugs you need, when you need them. That's the beauty of HEALTHMILES PLUS.

Overview of benefits

Low Deductible Package

$1,000 Single Deductible
Consider this package if you want to limit your out of pocket expenses with a lower deductible and a higher monthly premium.

• Covered expenses* – Pays 80 percent of covered costs for in-network providers and 60 percent of covered costs for out-of-network providers once the deductible has been met. If you seek services from out-of-network providers you are subject to a higher deductible.
• Prescription benefit (called RxImpact) – Provides an allowance of either $30/20/10/0 toward your prescription with a maximum of $100 per prescription; and a $2,500 annual out-of-pocket limit to member.
• Office visit copayment – Covers 100% after $25 copayment for a primary care visit or a $40 copayment for a specialty care visit for up to 4 visits per family member annually. This benefit does not apply to preventive care.
• Preventive care – Provides up to $300 of coverage annually for services such as routine annual physical exams and routine immunizations. Waiting period applies.

Medium Deductible Package

$2,000 Single Deductible
Consider this package if you want a lower monthly premium and a slightly higher deductible.

• Covered expenses* – Pays 80 percent of covered costs for in-network providers and 60 percent of covered costs for out-of-network providers, once the deductible has been met. If you seek services from out-of-network providers you are subject to a higher deductible.
• Prescription benefit (called RxImpact) – Provides an allowance of either $30/20/10/0 toward your prescription with a maximum of $100 per prescription; and a $2,500 annual out-of-pocket limit to member.
• Office visit copayment – Covers 100% after $25 copayment for a primary care visit or a $40 copayment for a specialty care visit for up to 4 visits per family member annually. This benefit does not apply to preventive care
• Preventive care – Provides up to $300 of coverage annually services such as routine annual physical exams and routine immunizations. Waiting period applies.

High Deductible Package

$3,000 Single Deductible
Consider this package if you want coverage with the lowest monthly premium or are interested in opening a Health Savings Account (HSA).

• Covered expenses* – Pays 100 percent of covered costs for in-network providers and 70 percent for out-of-network providers once the $3,000 deductible has been met. ($3,000 per person, with family options available.)
• Prescription benefit – After deductible has been met, pays 100% of covered prescriptions purchased at in-network pharmacies and pays 70% or covered prescriptions purchased at out-of-network pharmacies.
• Preventive care – Provides up to $300 of coverage annually for services such as routine annual physical exams and routine immunizations.
